Filling in phone numer is optional, but highly recommended. Phone contact is used always when fast information exchange is needed. Phone numer will be used only in reference to Conference attendance
Extra information

Students have to show valid Student ID at the registration desk. PTB, ESB and ISB members have to have valid memebrship (memebrship fee paid before the end of March).

  1. The Data Protection Officer (DPO) appointed by the Administrator supervises the correctness of personal data processing. You can contact the Data Protection Officer via traditional mail by writing to the following address: ul.   2. Your personal data will be processed in order to conduct the conference registration process and preparation of the invoices.
  3. The processing of personal data requires a legal basis, which is Article 6 par. 1 indent a and f of the GDPR, 2016/679 from 2016/04/27.
  4. Providing data is voluntary, but necessary to achieve the objectives for which they are collected.
  5. Access to your personal data will be available to authorised employees and associates of the Medical University of Warsaw. Recipients of personal data may also include the entities authorised by law: IT system operators, payment system operators, SMS system operators, email marketing system operators, payment operators, couriers and other subcontractors.
  6. Your Personal data will be processed for the period necessary to administer your participation in the conference or if necessarily for the period of 5 years.
  7. If the processing of your data takes place on the basis of your consent, you have the right to withdraw your consent at any time by sending such request for the e-mail address of Organizing Committee or : iod@wum.edu.pl.
  8. With regard to the processing of your personal data, you have the right to: access personal data; correct personal data; delete personal data; limit the processing of personal data; appeal against the processing of personal data; transfer personal data; lodge a complaint to the supervisory body, withdraw a consent at any time.
  9. You have right to file a complaint to the President of the Personal Data Protection Office.
  10. Your personal data will not be processed by automated means, including those based on profiling.
