Consent to personal data processing *

I consent to the personal data processing by Medical University of Warsaw at Żwirki i Wigury Street 61, 02-091 Warsaw, in order to take actions necessary for participation in BIOMECHANICS 2020 Scientific Conference. I declare, I was informed about my rights and obligations contained in information obligation, which is an integral part of this consent. I declare that I gave personal data freely. I declare that sending an electronic form is equivalent to the consent.

I consent to the use of picture(s), image(s), video(s) or photograph(s) (collectively referred to as the “Image”) of me, according to the act about copyright and related rights of 4th February 1994 (Dz. U. z 2017r. poz. 880) for informative and promotional purposes connected with the preparation and conduct of the BIOMECHANICS 2020 Scientific Conference through dissemination of it via the Organizer’s website and it’s social media profiles and through any other materials connected with the Conference. The Administrator of your personal data is the Medical University in Warsaw at ul. Żwirki i Wigury 61, 02-091 Warsaw.

The Data Protection Officer (DPO) appointed by the Administrator supervises the correctness of personal data processing. Your personal data will be processed in order to conduct the conference registration process and preparation of the invoices.
The processing of personal data requires a legal basis, which is Article 6 par. 1 indent a and f of the GDPR, 2016/679 from 2016/04/27.
Providing data is voluntary, but necessary to achieve the objectives for which they are collected.
Access to your personal data will be available to authorised employees and associates of the Medical University of Warsaw. Recipients of personal data may also include the entities authorised by law: IT system operators, payment system operators, SMS system operators, email marketing system operators, payment operators, couriers and other subcontractors.
Your Personal data will be processed for the period necessary to administer your participation in the conference or if necessarily for the period of 5 years.
If the processing of your data takes place on the basis of your consent, you have the right to withdraw your consent at any time by sending such request for the e-mail address of Organizing Committee or : iod@wum.edu.pl.
With regard to the processing of your personal data, you have the right to: access personal data; correct personal data; delete personal data; limit the processing of personal data; appeal against the processing of personal data; transfer personal data; lodge a complaint to the supervisory body, withdraw a consent at any time.
You have right to file a complaint to the President of the Personal Data Protection Office.
Your personal data will not be processed by automated means, including those based on profiling.
